What is the definition of the word tumescent?&lt;br /&gt;The word tumescent means swollen and firm.&lt;br /&gt;2.)  What is the tumescent technique?&lt;br /&gt;The tumescent technique for liposuction involves the injection of large volumes of very dilute lidocaine (local anesthetic) and epinephrine (drug that shrinks capillaries) into subcutaneous fat.  With the tumescent technique, the volume of dilute lidocaine that is injected into fat is so large that the targeted areas literally become tumescent (swollen and firm).  The tumescent technique produces profound and long-lasting local anesthesia of the skin and subcutaneous fat.  Tumescent liposuction designates a form of liposuction that uses tumescent local anesthesia.&lt;br /&gt;3.)  What is tumescent liposuction?&lt;br /&gt;The word tumescent refers to anything that is swollen and firm.  In the tumescent technique for liposuction, a large volume of very dilute solution of local anesthesia (lidocaine and epinephrine) is infiltrated (injected) into the fat beneath the skin, causing the targeted area to become tumescent.  The drug epinephrine (adrenaline) provides profound localized vasoconstriction that is virtually eliminates surgical bleeding during tumescent liposuction. &lt;br /&gt;4.)  What are the benefits of tumescent liposuction?&lt;br /&gt;As a result of the widespread capillary constriction caused by the epinephrine in the anesthetic solution, there is minimal bleeding during and after surgery. &lt;br /&gt;5.)  How much blood is lost during tumescent liposuction?&lt;br /&gt;The tumescent technique is so effective at minimizing blood loss that the jmajority of patients lose approximately 15 to 30 ml (1 to 2 tablespoons) of blood during large volume liposuction.  This is the same volume of blood that is taken for routine pre-operative laboratory sutudies.  In the days before the advent of the tumescent technique, the biggest risk of liposuction was excessive loss of blood during surgery.  In fact, many surgeons required their patients to donate their own blood several weeks before surgery, and the during surgery, the blood would be returned to the patient by transfusion.  With the advent of the tumescent technique, blood transfusions are no longer a part of liposuction surgery.&lt;br /&gt;6.)  How does the tumescent technique reduce bruising after liposuction?&lt;br /&gt;There are two reasons there is dramatically less bruising with tumescent liposuction than other techniques.  First, because there is so little blood loss with the tumescent technique, there is almost no blood that remains beneath the skin to cause bruising after surgery.  The second reason for decreased bruising is that with the tumescent technique there is a considerable amount of post-operative drainage of the blood tinged anesthetic solution the first 24 hours following surgery. In addition, breast reconstruction after preventive mastectomy was as safe as or safer than reconstruction in women with breast cancer and had excellent cosmetic results.&lt;br /&gt;&lt;br /&gt;"Breast cancer is a terrible diagnosis and decisions regarding treatment are never easy. This study shows that women with cancer in one breast who choose to have their other breast removed as a preventive measure are happy with their decision and a high percentage would do it again," said Scott Spear, MD, study co-author and past ASPS president. "More remarkable is the 100 percent satisfaction level, as well as the 100 percent willingness to have breast reconstruciton again, for the women who chose to have both breasts removed."&lt;br /&gt;&lt;br /&gt;The study examined 74 women who had preventive mastectomies and subsequent breast reconstruction between 2000 and 2005. Forty-seven patients had breast cancer in one breast and elected to surgically remove their other breast (unilateral prophylactic mastectomy). Twenty-seven patients did not have breast cancer, but chose to surgically remove both breasts due to a high-risk of developing breast cancer (bilateral prophylactic mastectomy). The cosmetic outcome was scored by 14 surgeons who looked at post-reconstruction photos and evaluated the result on a 1 to 4 scale (4 being an "excellent" result).&lt;br /&gt;&lt;br /&gt;The study found that women who had a bilateral prophylactic mastectomy were 100 percent satisfied with their breast reconstruction and 100 percent of them would have the surgery again. Ninety-four percent of women who had unilateral prophylactic mastectomy were satisfied with their reconstruction and 96 percent of them would have reconstruction again.&lt;br /&gt;&lt;br /&gt;The complication rate for reconstruction in women who had bilateral prophylactic mastectomy was 3 percent and 10 percent for those who had unilateral prophylactic mastectomy. Additionally, the study noted the cosmetic assessment for all patients was a score or 3 of 4.&lt;br /&gt;&lt;br /&gt;"These women look and feel the same or better and their risk of cancer has been taken off the table," said Dr. Spear. "For women who know they are at risk, this option gives them the opportunity to be active about their health and appearance rather than reactive. They can have excellent cosmetic results, low surgical risk and a high level of satisfaction with their breast reconstruction. This is empowering for women."&lt;br /&gt;&lt;br /&gt;According to ASPS statistics, more than 57,000 breast reconstructions were performed in 2007, up 2 percent since 2006.&lt;br /&gt;&lt;br /&gt;The American Society of Plastic Surgeons is the largest organization of board-certified plastic surgeons in the world. Making sure that your procedure can be performed in an accredited facility will significantly reduce safety risks and increase your comfort level. In fact, 40 percent of that 60 percent reported an annual income of $60,000 or less. Just 10 percent of respondents reported a household income of more than $90,000.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Myth 10&lt;br /&gt;&lt;br /&gt;All board certifications are the same.&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;Any group of medical professionals can create its own “certification” board. Most common procedures men are interested in are Botox, microdermabrasion and liposuction.&lt;br /&gt;&lt;br /&gt;Botox® is a botulinum toxin used to temporarily paralyze nerves within one’s facial appearance. Men may choose to have this safe, non-invasive procedure done to reduce the lines that may furrow their brows, or forehead. Microdermabrasion is another common procedure used to improve the appearance of one’s skin by removing areas of damaged or wrinkled skin. Liposuction, a surgical procedure used to removed unwanted fat from the hips, legs, arms or backs can create a more youthful and athletic physique for men.&lt;br /&gt;&lt;br /&gt;“Men are feeling more comfortable with being invested in their appearance,” says Dr. Barlow. “They notice how great their wives look, or they wish they looked differently, and aren’t afraid to ask how to gain back their younger-looking selves.”&lt;br /&gt;&lt;br /&gt;As the population ages, many successful, vibrant men are re-thinking their attitudes to dieting, face-care products and cosmetic surgery. Wanting to look youthful and appear dynamic well into one’s 40s, 50s and 60s means that some men will turn to a trusted and experienced plastic surgeon like Dr. Barlow for advice on how to regain a youthful appearance. &lt;br /&gt;&lt;br /&gt;From 2000 to 2005, there was a 44 percent increase in minimally invasive cosmetic procedures among men, according to the American Society of Plastic Surgeons. Minimally invasive cosmetic procedures are those that do not require surgery and therefore mean a quicker return to the office and everyday life.&lt;br /&gt;&lt;br /&gt;“Men are seeking out those treatments that will affect the way they look with the least possible fuss,” says Dr. Barlow. “They want to try something that is not a huge investment in time, like Botox. But they are also seeking out information on male breast reduction surgery, also known as gynecomastia, calf-implants and liposuction. Most of the time, they come in knowing what they want. But after spending some time discussing their goals, I can suggest some other procedures that may fit better with their expectations.”&lt;br /&gt;&lt;br /&gt;For most men, discussing these procedures may at first feel uncomfortable. Dr. Barlow has three suggestions for men when discussing what their goals are for plastic surgery:&lt;br /&gt;&lt;br /&gt;1. Identify exactly which physical attributes are making you uncomfortable. Do you wish you looked slimmer, more youthful or would you like to change the shape of your ears, nose or chest? Do you have a clear goal in mind? These will help your plastic surgeon determine the best plan of action.&lt;br /&gt;&lt;br /&gt;2. Bring in a picture of yourself when you were younger, or a celebrity whose looks and physique you admire. This will help guide both you and your plastic surgeon toward your ultimate goal.&lt;br /&gt;&lt;br /&gt;3. Keep an open mind about the procedures and techniques you are willing to try. Dr. Barlow’s number one goal with all his patients is to be sure they understand the benefits of each procedure, so they can make an informed decision about how to proceed.&lt;br /&gt;&lt;br /&gt;Looking to the future, Dr. Barlow says he expects tummy tucks and body lifts to become even more popular among men. Now through minimally-invasive surgical options we can bring about quite a transformation for ideal patients.&lt;br /&gt;&lt;br /&gt;For instance, chin and neck liposuction procedures are a unique form of facial rejuvenation that do not take a long time in the operating room or have a prolonged recovery period. Despite its minimally invasive nature, this surgical procedure can make a dramatic change in a person’s appearance. In the past, patients would need a facelift or neck lift to correct the problem of excess fat under the chin and in the neck. But, for appropriate candidates, this procedure is effective, less expensive, affords minimal recovery time, and successfully addresses the problem of the annoying "double chin" or overly full-looking neck. This procedure, which is ideal for both men and women, can dramatically improve a person’s appearance with minimal downtime and less risk of scarring. &lt;br /&gt;&lt;br /&gt;In the case of upper arm liposuction, ideal patients can benefit from the area between the elbow and the underarm being reshaped to result in a more toned and proportionate appearance.  This is can be a particularly troublesome spot for many patients to tackle.  Candidates with the appropriate skin elasticity are able to enjoy smoother skin and contours without having to undergo a full Brachioplasty (Arm Lift) procedure which is more invasive and can cause rather unsightly scarring in a highly visible location due to the large incisions necessary.  Similarly, addressing excess pockets of fat above the knee with liposuction is often a quick and relatively easy resolution to a pesky problem area.  &lt;br /&gt;&lt;br /&gt;The fact that any of these procedures can be performed individually or can be added on to surgery that you may already have planned is rather advantageous. However, in the case of cosmetic surgery, it is a necessary step in working toward a patient’s ideal final outcome.  Some board-certified plastic surgeons limit the cosmetic surgery procedures they perform on smokers because smoking increases the risk of pulmonary complications and can impede wound healing, which may decrease the quality of surgical results.&lt;br /&gt;&lt;br /&gt;In a survey by the University of Texas Southwestern Medical Center, 955 board-certified plastic surgeons were asked about their standards regarding treatment of patients who smoke.  While ninety percent of the board-certified plastic surgeons surveyed said they perform elective plastic surgery on smokers, approximately fifty percent of this group stated that they limit the type of procedure they perform on smokers. &lt;br /&gt;&lt;br /&gt;With many patients looking to their surgeon for superior skill and mastery of the most advances techniques and technology, patients who smoke are not always able to take advantage of these assets.  Two thirds of the surgeons surveyed said they "usually" or "always" perform a less invasive technique on smokers. While a third of respondents said they "sometimes" change their technique.&lt;br /&gt;&lt;br /&gt;Of the countless substances contained within tobacco smoke, nicotine and carbon monoxide are two of the most detrimental to wound healing and may cause pulmonary complications.   These substances have a harmful effect on tissue oxygenation, which impairs the reparative processes of wound healing and by delaying the formation of healing tissue, the stage is set for increased scarring at the edges of the wound.  It is this tendency to hinder our bodies wound healing response that may dramatically decrease the final cosmetic outcome.  At the same time, smoking is associated with an increased risk of wound infections due to the weakening of neutrophil defense against pathogens.&lt;br /&gt;&lt;br /&gt;In order to aid smokers reduce the risk of complications, Dr. Barlow asks patients to quit smoking prior to surgery. It has been shown that four weeks of abstinence from smoking reduces risk levels to those of non-smoking patients, regardless of the use of nicotine patches. While there are many things that I consider when faced with such a question, here are a few characteristics that may make you a good candidate for a facelift:&lt;br /&gt;&lt;br /&gt;1.       Do you have excess skin or true pouches and bags?  Excess skin can only be improved by reducing it surgically.  A filler can smooth pouches and take up some extra skin, but it cannot eliminate true bags.&lt;br /&gt;&lt;br /&gt;2.       Have your facial muscles descended or become displaced, in other words, has your neck bottomed out?  Muscles can descend due to aging and gravity.  Only a surgical lift can restore them back to a more youthful position, whereas fillers do not address the muscle at all.&lt;br /&gt;&lt;br /&gt;3.       Do you need fat reduced or redistributed?  If you have fat pooling in areas such as the brows, under the eyes, at the folds around the mouth, or even at the jaw line, a facelift is in order.  The liquid lift can enhance volume or relax expression lines, but it cannot remove fat or move it back where it once was.&lt;br /&gt;&lt;br /&gt;It is important to remember that there are a variety of facelift techniques that are employed to address each patient’s specific needs.  Similarly, great strides have been made in pre-operative preparation and post-operative care to enhance not only our patience results, but also their level of comfort during the healing process. The FDA’s decision to approve these implants was based on a thorough review of each company’s clinical and preclinical studies, a review of studies by independent scientific bodies and deliberations of advisory panels of outside experts that heard public comment from hundreds of stakeholders. The FDA also conducted inspections of each company’s manufacturing facilities to determine that they comply with FDA’s Good Manufacturing Practices.&lt;br /&gt;&lt;br /&gt;The director of FDA’s Center for Devices and Radiological Health, Daniel Schultz, M.D., stated “the silicone breast implant is one of the most extensively studied medical devices.” In the past decade, numerous independent studies have examined whether silicone gel-filled breast implants are associated with connective tissue disease or cancer. The studies, including a report by the Institute of Medicine, have concluded there is no convincing evidence that breast implants are associated with either of these diseases. Now that the products have been determined to be safe and effective, FDA will continue to monitor them by requiring each company to conduct a large post approval study following about 40,000 women for 10 years after receiving breast implants.&lt;br /&gt;&lt;br /&gt;While this announcement only confirms what those who have been working with silicone implants have been saying for years, we are glad that it removes any lingering doubt on the subject. This is excellent news for all of my patients, as it provides those who have already undergone breast augmentation surgery or reconstructive breast surgery with silicone implants with a great deal of reassurance. For those of you who have been considering the procedure, this provides you with a large body of new research to help you make your decision, and ultimately a sound alternative to saline implants. Ears are almost fully grown by age four, and the earlier the surgery, the less teasing and ridicule the child will have to endure. Ear surgery on adults is also possible, and there are generally no additional risks associated with ear surgery on an older patient.&lt;br /&gt;&lt;br /&gt;In the first consultation, Dr. Barlow will evaluate the patient's ears, and recommend the most effective technique. He will also give specific instructions on how to prepare for surgery.&lt;br /&gt;&lt;br /&gt;Otoplasty is commonly performed in our office, under local anesthesia. In the case of small children who might be uncomfortable being awake during surgery, it is also possible to have the procedure performed in the hospital under general anesthesia. Electrolysis, while effective and ideal for white, grey and blonde hairs, often requires years of ongoing and frequently painful treatments. All of these methods often result in scarring, allergic reactions, and ingrown hairs.&lt;br /&gt;&lt;br /&gt;Our patients are treated using the Candela GentleLASE system for hair removal to safely reduce unwanted body hair. Laser hair removal works by targeting the pigment inside the hair follicle without stimulating growth or damaging the delicate structures of the surrounding skin. Hair must have pigment to absorb the laser light. Therefore, gray, blonde and red hair colors are more difficult to treat. Because hair cycles through "growth" and "dormant" phases, we recommend a series of five treatments before long-term results are established. Maintenance treatments are often necessary.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;What does the treatment involve?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;You will be asked to wear eye protection to shield your eyes from the light of the laser. A small hand piece is placed against the skin and the laser is activated. After each laser pulse, a cooling spray is delivered to decrease any discomfort and to keep the skin from overheating. The hand piece is repositioned and the laser is activated again. The size or the area treated will determine the number of laser pulses required.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;How many sessions are needed?&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;The number of sessions will vary for each individual. This is dependent on the color, size and location of the unwanted hair. During the initial visit, the laser light disables those follicles in the active phase of the growth cycle. Follicles in the dormant phase will not be permanently affected at that time. Since follicles cycle through active and "dormant" phases, additional sessions may be desired once the dormant follicles become active. For this reason, multiple treatments will be necessary. Regrowth is usually lighter and thinner. Once the decreased density is achieved, maintenance treatments are recommended approximately once a year.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Is the laser treatment painful?&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;While some areas of the body are more sensitive than others, most patients report manageable discomfort. The Dynamic Cooling Device (DCD) attached to the laser sprays liquid cryogen onto the skin simultaneously with each laser pulse to alleviate the slight sting of the laser pulse. This system minimizes the level of discomfort during treatment.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Are there any side effects?&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;Immediately after the treatment the skin will look pink and slightly swollen, similar to a sunburn. This will subside within an hour or two and the skin will return to normal.&lt;br /&gt;&lt;br /&gt;Occasionally, a patient will have temporary pigment changes, blistering, or scabbing.&lt;br /&gt;As with electrolysis, cases of scarring have been reported with laser hair removal, but these are extremely rare. It can result in terrible embarrassment, teasing, and social trauma. Potential causes include puberty, steroid use, obesity, marijuana use, and genetic disorders.&lt;br /&gt;&lt;br /&gt;Breast growth is directed exclusively by female hormones--estrogens. Although men have some estrogen in their system, it is usually insufficient to cause much breast enlargement because it is counterbalanced by male hormones--androgens. Upsetting the balance, either by more of one or less of the other, results in the male developing female characteristics, breast growth being foremost. Failure of androgen productions can cause gynecomastia. Cirrhosis of the liver and lung cancer increase estrogen production as well.&lt;br /&gt;&lt;br /&gt;Male breast reduction corrects over-developed male breast tissue, creating a flatter, firmer, and more masculine chest contour. Usually, the fat deposits are removed by liposuction through a small incision along the line of the nipple. Persistent excess glandular tissue, fat, and skin can be surgically removed from around the areola and from the bottom and sides of the breast.&lt;br /&gt;&lt;br /&gt;Swelling and bruising are normal and will subside over the next few weeks. You will need to wear an elastic bandage or pressure garment as you heal. Scarring is usally minimal and will fade with time. Although both narcotics and the pain pump result in less pain and continuous pain relief, the On-Q pump offers benefits narcotics do not: faster return to normal activities, quicker return to normal body function, clear and groggy-free head, greater mobility, and more comfortable recovery. Also, with narcotics there is always a risk of nausea, vomiting, constipation, slower recovery, a groggy feeling, potential for addiction, and a higher risk for intense pain spikes. The pain pump provides local anesthetic pain relief to the surgical sites, without affecting the rest of your body.&lt;br /&gt;&lt;br /&gt;Q: How does it work?&lt;br /&gt;&lt;br /&gt;A: The pump is attached to a catheter near your incision site. Some women who seek breast augmentation have small breasts and want to balance out their figures. Others have lost breast tissue through aging or childbirth, and some would like to correct breast asymmetry which may be congenital.&lt;br /&gt;&lt;br /&gt;Implants can be placed above or below the muscle. Placement is based on several factors: the appearance of your breasts at the time of the consultation, the size of the implant to be used, and desired/expected result. In a sub-glandular placement, the implant is placed over the pectoral muscle and under the breast tissue. This provides for a shorter recovery, but since it is closer to the surface, there is a greater chance that the implant will be more visible under the breast tissue. With sub-muscular placement, the implant is place below both the pectoral muscle and the overlying breast tissue. This area offers less risk of implant visibility and hardened scar tissue around the implant [capsular contracture] and less interference for future mammography. This implant placement requires a slightly longer recovery time to accommodate the stretching of the muscle.&lt;br /&gt;&lt;br /&gt;Special consideration is taken in selecting the size of the implant. Since the implants come in measurements of volume [cc’s], not cup sizes, Dr. Barlow has samples/sizers available in the office so you can try several on to see which provides the best fit.&lt;br /&gt;&lt;br /&gt;Dr. Barlow uses both saline and silicone implants. Saline implants have been FDA-approved since 1992. They can be pre-filled to a specific size or filled at the time of placement, which has the advantage of allowing smaller incisions to insert the implant and minor size adjustments are made during placement. If a saline-filled implant leaks or ruptures, it is readily noticeable because breast volume will diminish rapidly. The saline is completely absorbed by the body and naturally eliminated. Saline implants can feel firmer than natural breast tissue, particularly if they are filled to shell capacity. Silicone implants can be used in limited situations. These implants come pre-filled and require a slightly larger incision. Although FDA approval is still pending, these implants are a possibility if the patient meets certain criteria and agrees to participate in an on-going study.&lt;br /&gt;&lt;br /&gt;There are two main incisions that Dr. Barlow uses: peri-areolar and inframmatory. The peri-areolar incision is made around the areola outside the edge of the nipple. This incision is usually made around the inferior or lower part of the areola skin where it joins with breast skin. When this incision heals there will be a fine white line that is hard to detect upon exam. The inframmatory incision is made in the fold under the breast tissue. This incision gives the best access to the breast tissue. When it heals well this incision will be a fine line that is hard to detect upon exam. It is hidden unless you show someone.&lt;br /&gt;&lt;br /&gt;Breast augmentation is performed under general anesthesia in the ambulatory surgery center of Fairfax Hospital, or at Woodburn Surgery Center. All anesthesiologists are board certified. The procedure usually takes about an hour to an hour and a half.&lt;br /&gt;&lt;br /&gt;Recovery time for the procedure varies per patient. You will be up and walking around the day of surgery, but will need a responsible adult to care for you and monitor your condition for at least 24 hours. Generally, you can remove the dressings and go into a non-underwire sports bra several days after the procedure, as well as shower. Most people are able to return to work within a week [performing non-physical tasks] and return to light exercising after 3-4 weeks. Scars begin to fade within four to six weeks.
